Statewide positions available! NFI North's Care Management Entity (CME) has openings for Care Coordinators to assist youth and families in the Transitional Enhanced Care Coordination (TrECC) program. This is a hybrid position working from a home office with occasional travel to the CME office locations in Tilton, Littleton and Portsmouth, NH. This position requires regular travel across New Hampshire, and throughout New England to support youth who are residential settings. The TrECC program utilizes ongoing collaborative analysis and implementation science to improve practice and ensure best outcomes for families as the program continues to develop and grow.

This position includes duties such as:

The TrECC Care Coordinator will facilitate regular meetings with families and teams, utilize graphic facilitation, oversee the development of TrECC Service Plans, support development of crisis/safety plans, and complete necessary progress monitoring tools and assessments.

The TrECC Care Coordinator will identify and engage natural supports, maintain ongoing contact with youth and families in their homes, residential treatment episodes, and in their community. They will also advocate for the needs of youth and families as appropriate and coordinate the transition of services as needed.

The TrECC Care Coordinator will attend regular supervision, engage in professional development activities, maintain documentation of treatment and services for each assigned youth, and be available to support and connect youth and families to appropriate resources during times of crisis.

The TrECC Care Coordinator will perform all duties in accordance with the agency's policies and procedures and ensure compliance with Joint Commission, federal, state and local regulations and standards.

The candidate would be detail oriented, flexible, able to multitask, have excellent verbal, written, and computer skills (to include but not limited to expertise in Microsoft Word and Excel and experience with electronic health record system), and be able to collaborate well with multiple partners and outside resources.

Qualifications: A Master's degree is preferred and 2 years of experience with high risk populations.
